A Merrill Lynch self-directed, non-interest bearing brokerage account, called a Limited Individual Investor Account (LIIA), was recently opened on your behalf. This account was opened for you upon the purchase of Waste Management stock through the Employee Stock Purchase Plan (ESPP).

To provide your certification, please fill out the Limited Individual Investor Account Form for U.S. participants or international participants, and sign and date it where indicated. Print the form and mail it to the Merrill Lynch address listed on the last page. Or, if you prefer, you can fax the form to Merrill Lynch at 1.609.274.0333. Note that when a W‐8BEN form is approved, it remains valid for the rest of the year plus the following three calendar years.
